ICSI Treatment & Its 6 Stages

ICSI stands for intracytoplasmic sperm injection. The procedure is based on the semen retrieval of a male. In this procedure, the best sperm is selected and then artificially injected with the help of a special instrument into the egg for fertilization. More than half of couples who try ICSI as a treatment option are able to enjoy a parenthood journey.

The treatment is used for couples who have no issues with fertility but suffer from infertility problems due to low sperm count or poor quality of sperm. These are couples who have previously tried to conceive through other means. It is a process that greatly increases fertility chances in men with low counts of healthy sperm.

ICSI procedure has successfully treated male factor infertility since its inception in 1992. The process has been refined over time, and studies have shown an increase in birth rates with ICSI. Many hospitals and clinics in Chandigarh provide the best ICSI treatment.

ICSI is not a one-day process. Generally, there are six major stages of intracytoplasmic sperm injection. We will discuss all of them in detail in this article. Let us start without wasting time.

5 Key Stages Of ICSI Treatment:

  1. Down-Regulation

This is the very first stage. Like other infertility treatments, ICSI starts with down-regulation. It involves suppressing the female’s natural menstrual cycle using medication to enable doctors to control ovulation and monitor follicular development.

This stage aims to stimulate the ovary, favor the preparation of a good number of oocytes, and reduce the number of follicles to improve the results and ensure their quality. This is further backed by birth control pills and progesterone supplements which help prevent the release of eggs by the ovaries before they are ready to be fertilized. The injection can be given subcutaneously or intramuscularly, and you will have to take it daily for 10 days.

  1. Ovarian Simulation

The next stage involves administering another injection called Gonadotropin-Releasing Hormone (GnRH) agonist. It helps to stimulate the ovaries so that they produce multiple eggs. You will also have to take injections of follicle-stimulating hormone (FSH) during this period. This will help you develop multiple mature eggs for fertilization. These drugs are usually administered through injections and can significantly raise estrogen levels in women, making them feel nauseous, bloated, or even cause mood swings.

  1. Egg Collection

In order to perform the ICSI, eggs are collected from your ovaries surgically by using a needle. This is done under anesthesia. You will be unconscious during the entire process. The follicles in your ovary will be punctured to collect the eggs. These eggs will be taken out intact. After collection, these eggs would be kept in an incubator that would provide them with an environment similar to what they experienced in your ovaries.

  1. Sperm Collection

The next step after egg retrieval is sperm collection. Sperm can be collected naturally or otherwise by taking it out through a needle aspiration procedure called testicular aspiration (TESA). Macro-surgical TESA and Microsurgical TESA are two different methods for collecting sperm through the TESA technique.

  1. Insemination

Insemination is the process in which sperms are injected directly into the egg at a microscopic level; ideally, this process should take place 36 hours after the egg collection so that eggs can be matured enough to receive sperm. The fertilized eggs are then placed back in a cell culture medium and left for 16-18 hours before checking for signs of fertilization.

  1. Embryo Transfer

After a few days, the embryos will be cultured and assessed by our embryologists when eggs get fertilized. Your specialist will advise you if we recommend transferring one embryo or two (or none at all).

If you are over 35 years old, we may transfer two embryos. If you are under 35 and have a very good chance of success, we may only transfer one. We can discuss this further with you during your consultation.

The transfer is a quick and simple procedure, much like a smear test. It is performed in our dedicated treatment room using an ultrasound scan to guide a fine plastic tube into the womb. Once in place, the embryos are gently deposited into the uterus through this tube.

The whole process takes less than 5 minutes, and no anesthetic is required. You can then go home shortly after the procedure but ideally should rest for the rest of the day.
